FY2016 FCC Form 470 now available

The FY2016 Form 470 is now available online via the E-rate Productivity Center (EPC). Paper versions of the Form 470 and its instructions will no longer be available. Applicants will sign and certify Form 470 online prior to being posted to USAC’s website. The Form 470 must be signed and certified before USAC starts the 28-day waiting period. Forms will automatically be routed to users within the organization who have the authority to sign them. Once Form 470 is submitted, a receipt notification will appear in the EPC News feed. USAC will no longer issue paper Form 470 Receipt Notification Letter. Corrections of any clerical errors can be submitted via EPC.
