The Schools and Libraries announced Nov. 2 that Year 4 E-rate applicants will be required to use a new version of the Form 471 when they submit their application, and that applications on the old version of the form will not be accepted.
The new version of the form adds a field to indicate the service end date for tariffed and month-to-month services, and removes an unnecessary column on the discount worksheets for libraries and consortia. It also modifies the "Minor Modification of Contract" section that is not completed when an application is submitted the first time.
The new form will be available on the SLD Web site and will be incorporated into the SLD's online filing mechanism.
