The Schools and Libraries Division has warned E-rate applicants who receive funding commitments in the first two waves of the 1999-2000 funding year to hold off filing their Form 486 until it makes a new version of the form available.
The new form, which will carry the date of May 1999, includes an additional certification that the applicant is seeking support only for signed contracts, unless the services are tariffed telecommunications services.
The new Form 486 is available online and will be mailed to applicants who received funding commitments in Waves One and Two.
