As part of the FCC’s effort to reduce paperwork burden on E-rate stakeholders and as required by the Paperwork Reduction Act of 1995 (PRA), the FCC is recommending to the Office of Management and Budget (OMB) that no revisions are necessary for the following forms:
- Form 472 Billed Entity Applicant Reimbursement (BEAR) Form
- Form 473 Service Provider Annual Certification (SPAC) Form
- Form 474 Service Provider Invoice (SPI) Form
The Commission would like public feedback on whether the accuracy of the time burden to complete the forms are accurate and possible additional ways to enhance the quality and clarity of the information collected. The FCC is looking for a three year clearance from the OMB and the PRA comments must be submitted by April 12, 2019.
