Service providers looking to submit their service provider invoice forms (Form 474) with a customer billed date of 2018 currently cannot submit invoices. The SLD online invoice system has not been updated to include calendar year 2018 dates. Below is a screen shot of the webpage on the SLD site. The form wizard is missing the year 2018 from the dropdown menu for the customer billed date section. Service providers should contact the USAC help line for additional assistance at 888-203-8100.
