The SLD confirmed that starting July 1, 2016, applicants who choose the BEAR payment method in order to receive reimbursement from the SLD on payments must complete and certify a revised Form 472. However, applicants must first complete an FCC Form 498 to provide contact and banking information to the SLD in order to facilitate the direct electronic deposits. This new requirement is part of the FCC’s 7th Report & Order.
Applicants can access Form 498 through the E-rate Productivity Center (EPC). The Form 498 must be processed by the SLD before July 1, 2016 in order to make direct deposit of the reimbursements. Further details can be found in the November 20, 2015 SLD’s E-rate News brief.
